§ 148E.095 — License Certificate

This section governs the license wall certificate that social workers receive when licensed. The board issues a wall certificate at no extra charge when a new license is issued. If a social worker needs a duplicate certificate—for example, due to a name change or damage—they must submit a written request and pay a fee.
148E.095 LICENSE CERTIFICATE.
§
Subdivision 1.License wall certificate.
(a) The board must issue a new license wall certificate when the board issues a new license. No fee in addition to the applicable license fee specified in section 148E.180 is required.
(b) To request a duplicate license wall certificate, the licensee must:
(1) submit a request for another license wall certificate in writing; and
(2) submit the license wall certificate fee specified in section 148E.180. §
