§ 148G.07 — Name Change and Change of Address

148G.07 NAME CHANGE AND CHANGE OF ADDRESS.
A certified midwife must maintain a current name and address with the board and must notify the board in writing within 30 days of any change in name or address. All notices or other correspondence mailed to or served upon a certified midwife by the board at the licensee’s address on file with the board are considered received by the licensee.

Practical Notes
Certified midwives who move or change their name must notify the Board of Nursing within 30 days. If you do not update your address and the board sends you an important notice, you are legally considered to have received it even if the mail does not reach you. Keeping your information current protects you from missing critical regulatory communications.
