Before you sign, a health plan company must give you a complete copy of the proposed contract, including attachments, manuals, coding guidelines, and the fee schedule (or a way to determine it). Changes that alter your fee schedule or materially change governing policies must be disclosed at least 45 days before they take effect, and you can terminate the contract first. On request, the company must also provide relevant fee schedules and, where provider tiering is used, an explanation of how your tier ranking was calculated.
