§ 144G.30 — Surveys and Investigations

Plain-Language Summary

The Department of Health is the sole state agency responsible for surveying and investigating all licensed assisted living facilities in Minnesota. The department must survey each facility at least once every two years, without advance notice. Facilities must provide accurate information and access to records during surveys. The department can issue correction orders when violations are found, and facilities must post and make available their plans to fix problems.

Practical Notes
Assisted living facility surveys happen without advance notice — you may have a surveyor show up at any time. Facilities must make all records available to the department and cooperate fully with surveys and investigations. Residents and families can request a copy of the facility’s most recent plan of correction to learn about any violations found and how the facility is fixing them.
