§ 256F.10 — Child Welfare Targeted Case Management

Plain-Language Summary

This section creates a Medicaid-funded child welfare targeted case management program for children under 21 who are at risk of foster care placement, at risk of or experiencing maltreatment, or in need of protection or services. Counties and tribal social services agencies provide these case management services, and eligible children can choose their certified provider. Federal Medicaid funds earned through this program must be used to expand preventive child welfare services.

Practical Notes
If your child is involved with child protection services or at risk of foster care placement, they may be eligible for this intensive case management service that coordinates all the resources they need. Certified providers include county social services departments and tribal social services agencies. A case manager assigned under this program must have a bachelor’s degree in a relevant field and help coordinate access to community resources. Federal funding flows through Medicaid, making these services available regardless of the county’s financial situation.
