§ 62A.4516 — Evidence of Coverage

Prepaid limited health service organizations must provide each enrollee with written evidence of coverage describing the benefits, limitations, and terms of the plan.
62A.4516 EVIDENCE OF COVERAGE.
Every subscriber must be issued an evidence of coverage consistent with the requirements of Medicare Part D.
