§ 144G.55 — Coordinated Moves

Plain-Language Summary

When an assisted living facility terminates a contract, reduces services significantly, or closes, it must ensure a 'coordinated move' — finding appropriate alternative housing and services for the resident, consulting with the resident and their representatives, and creating a written relocation plan. For metro area facilities, at least three alternative facilities must be identified; for outstate facilities, at least two. Residents can always choose to move to a location of their own choosing instead of the facility's suggestions.

Practical Notes
A facility cannot simply give you notice and leave you to figure out where to go. The facility must actively help you find a suitable new place, consult with your representatives and health professionals, and prepare a written plan. You do not have to accept the facility’s suggested alternatives — you have the right to choose your own destination. If a facility fails to do a coordinated move, report it to the Ombudsman for Long-Term Care.
